Description

Thousands of Veterans receive care in the community in both unscheduled urgent/acute care settings and scheduled care settings. Following these encounters, VA must receive clinical documentation describing the encounter and address follow-up care needs. The current process for this is manual, inefficient, and not always effective at timely documentation retrieval. This results in delayed care for Veterans and duplication of services.
