Community Care Records Retrieval
Description
VA care teams often struggle to access Community Care documents in a timely manner. When access is granted, the process is labor-intensive, requires significant manual administration by clinical staff, and is not always successful. Veterans may also be asked to bring in a copy of their records when VA providers are unable to obtain them. This negatively impacts Veterans’ care coordination and makes them lose trust in VA.
